To make a purchase request, you'll need to submit a Purchase Request Form to an LCVR Admin.

 

The following will need to be included on the form:

1. Date needed

2. Vendor

3. Link to item needed (if applicable)

4. Item description

5. Funding Source

6. Justification

7. PI signature
